USCIS Immigration Medical Exam
Taking the next step toward a U.S. Green Card or citizenship is a milestone worth celebrating and we're honored to be part of that journey. At Glow Pediatrics, our Board-Certified physician is a USCIS-designated Civil Surgeon authorized to conduct official immigration medical examinations in Oklahoma.

We complete two key USCIS certifications:​
Form I-693 — Required for Green Card applicants. Full physical exam, immunization review, and all required lab tests. Completed and sealed by our Civil Surgeon.

Form N-648 — For citizenship applicants with a medical condition affecting English or civics test requirements. Evaluated with care and fully USCIS-compliant.
What To Bring
To help avoid delays, please bring the following
ID
A valid Government-issued photo identification
Forms I-693 & N-648
These forms are provided at the office
Vaccinations
Proof of prior vaccinations or immunizations
Medical History
Your Medical History and a List of Current Medications.
Payment
Payment - Due at The Time of Your Visit (We Accept Cash, Debit and Credit Cards).
Interpreter
We welcome interpreters to ensure clear communication during your medical exam.
What To Expect During The Exam
Your visit includes all services required by USCIS:
A Complete Physical Examination.
Review of Your Vaccination Records
Administration of Required Immunizations (if Needed).
Tuberculosis (TB) Screening
Testing for Gonorrhea and Syphilis.
We recommend scheduling your exam just before submitting your I-485 application.
